Program Referral Specialist

The Institute for Human Services (IHS) is Hawaii's most comprehensive human services agency dedicated to ending and preventing homelessness. For over 45 years, we've been at the forefront of creating tailored solutions for those in crisis, offering a continuum of care that includes emergency shelters, housing programs, health services, employment assistance, and outreach.

The Program Referral Specialist provides administrative referral support and facilitates communication with internal and external partners that ensure hospital and outside referrals to IHS specialty shelters are successfully placed or resolved. This position updates and monitors data and reports for review by the Director of Operations to ensure successful outcomes, CARF and all contract compliance requirements are achieved.

Compensation:

Starting from $20.00 hourly, actual pay commensurate with experience.

Essential Functions:

  1. Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.
  2. Establish and maintain contacts with providers, particularly as they relate to program articulation and promotion.
  3. Function as one of the primary points of contact with hospitals to ensure timely and appropriate execution for referral correspondence, agency forms and processes.
  4. Implement IHS administrative procedures and practices for the program and assist in the preparation for and documentation of weekly provider meetings to ensure completion of action items and to establish a record of proceedings.
  5. Prepare and draft agendas for various provider meetings and other clinical/operational meetings.
  6. Communicate with the Referral Coordinator and across teams to resolve hospital referrals and dispositions.
  7. Maintain confidentiality and consistently maintain accurate attendance, safety and program records through SAMI and/or other systems as required; provide data and reports to demonstrate/support outcome measures.

Required Knowledge/Skills/Abilities:

  1. Working knowledge of care coordination and health care.
  2. Effective prioritization and problem solving skills and able to deal with challenging behaviors.
  3. Strong empathy skills with ability to bridge IHS operations and referring hospital care teams.
  4. Proficient in Microsoft Office including Excel, and database applications.
  5. Possess personal qualities of integrity and commitment to mission.
  6. Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Required Education and Experience:

  1. High School diploma or GED equivalent.
  2. At least one year of office experience in a fast paced service environment.
  3. Basic understanding of the issues affecting homelessness and healthcare coordination.

Position Type/Expected Hours of Work:

Regular Full-Time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
