CIS Housing Navigator (Case Manager)

The CIS Housing Navigator provides pre and post tenancy services for homeless clients. Responsibilities include, but are not limited to, CIS Referral form, Member consent, assessment/reassessment and CIS health action plan addendum providing education and support for the client and others in the client’s significant support network. This position links clients to community resources to further independence and stability. The CIS Housing Navigator will also provide quality case management to include supportive counseling, outreach, and provision of safe, decent and affordable housing opportunities, skills teaching/education, family support, service coordination/advocacy, and crisis support for CIS consumers.
Compensation:
Starting from $20.00 hourly, actual pay commensurate with experience.
Essential Functions:
- Demonstrate excellent customer service consistent with the organization's Mission and Vision while modeling organizational Values.
- Advocate on behalf of each client and ensure approvals; coordinate, monitor and evaluate services with collateral contacts that support access to with the appropriate programs, including substance abuse assessments and linkage to substance abuse treatment as appropriate.
- Manage a caseload and oversee the timely coordination of all services to include housing solutions.
- Provide face-to-face direct service contact for each client a minimum of two times per month or as directed/needed.
- Develop and ensure that each client has a current individual service plan; develop a transition/discharge plan for each client no longer meeting the criteria for the level of care.
- Provide supportive counseling or problem-solving activities to assist clients in developing and maintaining the support necessary to manage illness or addictions.
- Responsible for the integrity of the clinical record and consumer files including the completion and maintenance of all required clinical and administrative documentation according to IHS standards.
Required Knowledge/Skills/Abilities:
- Knowledge of community resources, levels of care, as well as the standards for care within the community resources; Case Management process and assessment and coordination.
- The ability to understand the benefit provisions of an insurer, the various types of payers, as well as the public funding programs to which a patient might be entitled to.
- Possess personal qualities of integrity, patience and commitment to mission.
- Flexible and able to multitask; can work within an ambiguous, fast-moving environment.
Required Education & Experience:
- High School Diploma or GED equivalent.
- One to two years of direct case management or supportive housing services and experience working with homeless or other disenfranchised individuals.
Position Type/Expected Hours of Work
Regular Full-Time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
